POSITION: Case Manager (Part-time/15 hrs per week)

Brief Description: Responsible for the completion of home visits, client assessments, referrals, counseling (case assessments and case management) for senior citizens 60 years of age or older. Initiate and maintain working relationship with other service providers throughout Ward 6 & 7 to identify the “at risk” elderly in the service area and to provide services to them, develop outreach plan for clients, assess referred clients to determine eligibility and need, oversee development of individual case management plan for clients, coordinating with other case managers, clients and their caregivers, health professionals and other provider, monitor clients’ progress and services provided, making changes when appropriate, provide supportive counseling for client and family member or caregiver when required, maintain weekly and monthly units of services, maintain compliance to confidentiality of information and usual standards of operation in delivering social services.
Minimum Qualification: Masters Degree in Social Work; licensed as Social Worker, 2 years experience working with geriatric population, 2 years experience working with senior citizens; creative self-starter, excellent communication skills with experience in communicating with and for senior citizens, must have own transportation with a valid driver’s license, demonstrate good oral communication skills, computer literate in MS Office.
This position IS in the collective bargaining unit represented by Communication Workers of America CWA). Any regular employee hired in a bargaining unit position, SHALL as a condition of employment, pay union dues and be a union member or pay amount equal to union dues within 30 days after entrance to UPO. ALL bargaining unit employees that are not paying union dues or an amount equal to union dues within 30 days will be terminated.
